 Child Referral

If you want to request an assessment for autism and/or ADHD for your child, this must be done by your GP or other health professional.

Here is the process for all new referrals for children and young people aged 5 – 15 with a GP in Hertfordshire.

Please contact us when you are ready to submit the completed forms. We will send you a link to enable you to attach the completed from with any supporting documents to your GP Practice.

What Happens After Referral?

  • Autism assessments are currently offered as a hybrid service, meaning they may be conducted face-to-face or virtually, depending on your child’s needs.
  • A clinician will contact you in advance to explain how the assessment will be carried out.
  • When your child reaches the top of the waiting list, you will receive forms via email that need to be completed and returned as soon as possible.

Referral – Adults

For adult autism referrals, please submit a patient triage form. Once received, we will contact you with the next steps.

Our team will determine if you meet the criteria for onward referral for a formal autism assessment.
